چکیده

The biomechanical factors influencing reduction of dislocated hips by the Pavlik harness in patients of Developmental Dysplasia of the Hip (DDH) were studied using a three-dimensional dynamic computer model capable of simulating hip reductions in (1) subluxated and (2) fully dislocated hip joints. Eight hip adductor muscles were identified as key mediators in the prognosis of hip dysplasia and the non-dimensional force contribution of each in the direction necessary to achieve concentric hip reductions was determined. Results point to the adductor muscles as the mediators of the reduction of 2 subluxated hips, as their mechanical action is a function of the degree of hip dislocation. For subluxated hips in abduction and flexion, the Pectineus, Adductor Brevis, Adductor Longus, and proximal Adductor Magnus contribute positively to reduction, while the rest of the Adductor Magnus contributes negatively. Furthermore, in full dislocations all muscles contribute detrimentally to reduction, elucidating the need for traction to reduce Graf IV type of dislocations. Reduction of dysplastic hips was found to occur in two distinct phases in which the muscles act distinctively: (I) release phase and (II) reduction phase. This is the first study that uses dynamic computer simulations to understand hip dysplasia reductions by the Pavlik harness, bringing to light conclusive findings that may be used to predict successful vs. unsuccessful treatments, allowing physicians to make better treatment decisions.

Mechanics of hip dysplasia reductions in infants using the Pavlik harness: a physics-based computational model.

Biomechanical factors influencing the reduction of dislocated hips with the Pavlik harness in patients of Developmental Dysplasia of the Hip (DDH) were studied using a three-dimensional computer model simulating hip reduction dynamics in (1) subluxated and (2) fully dislocated hip joints. Five hip adductor muscles were identified as key mediators of DDH prognosis, and the non-dimensional force ...

The costs of late detection of developmental dysplasia of the hip

PURPOSE Debate currently exists regarding the economic viability for screening for developmental dysplasia of the hip in infants. METHODS A prospective study of infant hip dysplasia over the period of 1998-2008 (36,960 live births) was performed to determine treatment complexity and associated costs of disease detection and hospital treatment, related to the age at presentation and treatment ...

Prolonged treatment with the Pavlik harness in infants with developmental dysplasia of the hip.

We prospectively studied the benefits and risks of prolonged treatment with the Pavlik harness in infants with idiopathic developmental dysplasia of the hip. Bracing was continued as long as abduction improved. It was started at a mean age of four months (1 to 6.9). Outcome measures were the number of successful reductions, the time to reduction, the acetabular index and evidence of avascular n...

Pitfalls in the use of the Pavlik harness for treatment of congenital dysplasia, subluxation, and dislocation of the hip.

We reviewed the records of treatment of eighteen infants with congenital dysplasia, subluxation, or dislocation of the hip who had problems with the involved hip following treatment with the Pavlik harness. The most common problem (seen in twelve patients with a dislocated hip) was failure to obtain reduction. This failure was attributed primarily to improper use of the harness by the physician...

Monitoring the treatment of developmental dysplasia of the hip with the Pavlik harness. The role of ultrasound.

We report the six-year results of a prospective, controlled demographic trial of developmental dysplasia of the hip (DDH) treated in the Pavlik harness using ultrasound supervision. Our aim was to assess the value of ultrasound and its role in monitoring reduction in the harness, in terms of progression or failure of reduction at an early state. From 1988 to 1994, a total of 221 patients with 3...
